Q:   How are children accepted into the IMPACT Plus Program?

A:   IMPACT Plus can be accessed in two different ways.

First, the parent or guardian or someone who works with the child
may contact us or another IMPACT Plus case management provider
to access a case manager.  The case manager facilitates the team
meetings for the child. The parent will always have the freedom to
choose any provider he or she feels is best for his or her child. This
right is referred to as "Freedom of Choice." Scroll to the bottom of this

Second, in some circumstances a child may already have a case manager. If so, the case manager can move ahead and develop an initial service plan at the direction of the child's team. The case manager can then present both the plan and request for eligibility to SHPS at the same time. 

Once the parent or guardian has given consent, the case manager will gather all necessary information and contact Medicaid's Peer Review Organization, SHPS, to determine the child's eligibility for IMPACT Plus services.

 

Q.  What are the requirements for participation in IMPACT Plus?

A.  Children who receive IMPACT Plus services must meet all of the following criteria:

They are Medicaid eligible. 

They are under 21 years of age. 


They are in the custody of the state, under the supervision of the state,
or at risk of being in the custody of the state.


They are at risk of institutionalization.


They have been diagnosed with a severe emotional disorder, and must be able document a severe behavioral health problem related to the diagnosis during the past six months.


They have been determined by a behavioral health professional to be at risk of continuing for six months.


They require a coordinated and intensive plan of medically necessary community-based behavioral health services. (These services are reasonably expected to improve the child's condition or prevent further regression, so that the child may be discharged to a less intense service or program in a six-month time frame).


One of the following must also be true:

 
A less intensive service will not meet the child's needs. 


An appropriate and less intensive service is not available or accessible. 


An appropriate and less intensive service is available and accessible, but a

severe concurrent diagnosis complicates the treatment of the primary diagnosis.
