| |  | | Targeted Case Management: A service that assists an IMPACT
Plus eligible recipient in accessing the needed medical, social,
educational, developmental and other services available in the
community. It is designed to be an intensive service. This service
is provided by a Targeted Case Manager or Service Coordinator.
Targeted Case Managers serve a very important function as they are
often the first line of contact for familes and youths who are seeking or
who are already involved in IMPACT Plus services. They aid in filling
out and submitting eligibility packets, conducting initial conversations with
families to orient them to the program, helping the family identify potential
team members, coordinating team meetings, and submitting requests for
intitial and ongoing services. Targeted Case Managers will actively monitor
service untilization, client progress and compliance, and fascilitate client
connection to available community resources that can best meet their
needs. The role of the Targeted Case Manager is central to each child's
treatment. |
